NOTICE: This information is regarding procedures for incidents only, where no medical treatment is necessary.
Employees who are involved in a safety incident, sustain a work-related injury or illness, or are diagnosed with a work-related disease must immediately notify their supervisor or manager, regardless of the severity of the incident or condition.
1. The supervisor completes this online form with the employee if this is a safety incident or first aid and no additional medical treatment is necessary.
2. No further action is required unless treatment is later deemed necessary. If treatment is necessary, please see the procedures for Filing a Claim.
